Understanding how HPP is diagnosed
An HPP diagnosis depends on 2 things:
Low alkaline phosphatase (ALP) levels over time
Experts identify low ALP over time as a hallmark of HPP—your doctor can order a simple blood test to track your ALP level.
Symptoms of HPP
HPP symptoms can affect different parts of the body. If symptoms are present along with low ALP over time, HPP can be confirmed.
Low ALP over time means 2 or more ALP levels below normal range at least 30 days apart. Your doctor may also perform other tests to get insight, help confirm a diagnosis, or support your care.

When I told my husband that I have been diagnosed with HPP, he said, ‘However we have to do it, we’ll do it.’ And he knows I don’t give up on hardly anything, and I didn’t give up on this.- CAROL, LIVING WITH HPP
Why do many people remain undiagnosed?
HPP can be hard to recognize because its symptoms—like headaches, brain fog, and pain in the joints or muscles—can show up in many conditions.
In fact, it’s even possible to have HPP along with other medical conditions—including ones that look like HPP, such as fibromyalgia, osteoporosis, arthritis, or pseudogout. Because of this overlap, HPP can sometimes look like other conditions.
What sets HPP apart is low ALP over time. While symptoms may overlap with other conditions, low ALP levels help distinguish HPP from other possible diagnoses. Seeing how HPP symptoms show up across the body can help bring the full picture into focus.

When the doctor confirmed we had HPP, I burst into tears. I was so relieved to cross that line. It was a significant hurdle to overcome.- SUZANNE, LIVING WITH HPP

Diagnosis of HPP in infants and unborn babies
Diagnosing HPP in infants
In infants, diagnosing HPP may involve imaging that looks at the whole body.
This imaging can show parts of the body that may not be developing appropriately due to HPP.

Diagnosing HPP in unborn babies
Diagnosis in the womb relies on ultrasound and other types of imaging.

Getting an accurate diagnosis is key
The sooner HPP is diagnosed, the sooner people living with the condition can get the care they need. Talk to your doctor about your symptoms. Ask if your ALP levels could point to HPP.
